How are Semen Samples collected? ♌️ The Semen Sample can be produced at the clinic or at home. Either way it is very important to use a STERILE SAMPLE CONTAINER as other containers can be toxic to sperm cells. Before producing the sample, you should wash your hands and genital areas to ensure good hygiene, as dirty samples may interfere in the interpretation of the results. If there’s any spillage of the sample, please inform the laboratory staff. This is important when interpreting the analysis as it may lead to inaccurate results. The sample should be produced as close as possible to the time of your appointment. If the sample was produced at home, it should be transported to the clinic at body temperature. You can achieve this by carrying it close to your body. Like for example, you could use the pockets of your jacket or your armpit. And If you are unable to produce a sample don’t worry. It also contains 5 to 25 calories and comprises small amounts of essential nutrients, including Calcium. Males produce millions of these cells daily, yet only one can fertilize an egg and create life! Is human sperm visible? Yes! but Only if you're looking through a microscope. Sperm are tiny. Like really tiny scientists can typically identify the following three parts When studying a sperm cell under a microscope The sperm head contains chromatin, the DNA material that makes up chromosomes. Typically, both the human sperm cell and the human egg cell each contain 23 chromosomes. When the sperm and egg combine, this results in an embryo cell with 46 chromosomes. Covering the head of the sperm is a cap called an acrosome. The acrosome contains enzymes that help the sperm penetrate the outer shell of an egg. The head shape is important because it can affect the sperm’s ability to penetrate the outer surface of a woman’s egg to fertilize it. The midsection of the sperm contains energy-producing mitochondria. These specialized structures provide the energy necessary for the sperm cell to move. The sperm tail is the most complicated structure, with more than 1000 proteins involved in its functioning. Usually it remains tightly closed, but here the cervix is open for a few days while the woman ovulates. The sperm swim through the cervical mucus, which has thinned to a more watery consistency for easier passage. Once inside the cervix, the sperm continue swimming toward the uterus, though millions will die trying to make it through the mucus. Some sperm remain behind, caught in the folds of the cervix, but they may later continue the journey as a backup to the first group. Inside the uterus, muscular uterine contractions assist the sperm on their journey toward the egg. However, resident cells from the woman’s immune system, mistaking the sperm for foreign invaders, destroy thousands more. Next, half the sperm head for the empty fallopian tube, while the other half swim toward the tube containing the unfertilized egg. Now, only a few thousand remain. Inside the fallopian tube, tiny cilia push the egg toward the uterus.To continue, the sperm must surge against this motion to reach the egg. Some sperm get trapped in the cilia and die. During this part of the journey, chemicals in the reproductive tract cause the membranes covering the heads of the sperm to change. As a result, the sperm become hyperactive, swimming harder and faster toward their destination.At long last, the sperm reach the egg. Only a few dozen of the original 300 million sperm remain. The semen analysis is designed to make sure that a male has sperm and to try to predict what the likelihood is of sperm reaching the Fallopian tubes where fertilization happens. There are several components to a semen analysis. Reading a semen analysis report can be confusing. Today on infertility TV we will explain a semen analysis report. Volume This tells you how much seminal fluid comes out in the ejaculate. By itself this is not a very helpful number for predicting fertility. The main reason why men may have a low semen volume is that they spilled some when trying to collect the specimen. Higher volumes are not associated with a better chance for getting pregnant. Concentration This is the most important number in the semen analysis. It tells you how many sperm there are. Men with very low sperm numbers definitely have more difficulty in achieving a pregnancy a normal sperm concentration is 20 million per milliliter however men with lower numbers may still be able to achieve a pregnancy or even have normal fertility. Higher numbers do not mean a better chance for getting pregnant. Motility Motility is a measurement of the movement of sperm. sperm have tails which whip back and forth very quickly to propel the sperm forward in the female reproductive tract. Some sperm may not move at all. Some sperm may move slowly or ineffectively. The net result is that less sperm will reach the fallopian tube and therefore the chance for fertilizing an egg is lower. The most important motility number is called Progressive motility. This is the percentage of sperm that have fast forward movement. These sperm are the ones most likely to reach the fallopian tube. Morphology This is a an estimate of the percentage of sperm with a normal appearance under the microscope. This is perhaps one of the most confusing parts of the semen analysis. This is due in part to the fact that there are different criteria for assessing morphology with different estimates for what is considered normal. These days most fertility specialists will use a criteria called strict morphology also known as the Kruger morphology after the physician who came up with it. All men even those with normal fertility will have some percentage of sperm with an abnormal appearance under the microscope. However men that have a very high percentage of abnormal appearing sperm have more difficulty producing pregnancies and their partners. It is not known precisely why their pregnancy rates are lower. There are two ways in which physicians might use these measurements. In some cases, the doctor may look at a specific number like motility and if it is abnormal, make certain recommendations for further testing or treatment. In other cases, doctors may calculate the total number of normal moving sperm by multiplying the volume times the concentration times the percent motility times the percent normal morphology. And if that falls below a certain number then those men are determined to be subfertile. Keep in mind that there is a tremendous amount of variability in these numbers.
