This can feel like a frustrating and continual processing or decoding of the world around them. Balancing these symptoms often leads to meltdowns where it feels like one is incapable of processing more sensory input or shutdowns where one needs to retreat to a silent place for an extended period of time before rejoining in normal activities. Many times these symptoms are imperceptible to others who are not autistic due to camouflaging or masking. Camouflaging/masking is the process through which autistic people consciously or subconsciously behave in order to appear more socially appropriate. This can affect body language, tone of voice, eye contact, word choice, and more. In de media berichten ligt vaak de nadruk op de nadelen (die er zeker ook zijn, want geen enkel model is allesomvattend). The DSM-5-TR was released in March 2022, nine years after the release of DSM-5. Although the medical model is not the only model that mental health professionals work with, it is an important one. Standards within the mental health professions (e.g., graduate program accreditation standards, best practice guidelines, codes of ethics) and from outside of the professions (e.g., licensure boards, third party payers) encourage or require mental health professionals to have an updated understanding of the DSM. Clinicians and forensic evaluators are busy professionals. Seguimi anche su facebook: 🤍 Il DSM è il Manuale diagnostico e statistico dei disturbi mentali redatto dall’American Psychiatric Association e, nello specifico, da una cosiddetta “task force” di professionisti nel campo della salute mentale il cui elenco trovate all’inizio del manuale. Siamo oggi alla quinta edizione che è stata pubblicata negli Stati Uniti nel 2013 e la versione italiana nel 2014. Attenzione, si scrive DSM-5 con il numero 5 e non con il numero romano V come veniva fatto per le precedenti edizioni. Il DSM è un sistema nosografico, per nosografico si intende un sistema descrittivo perché descrive i disturbi mentali sulla base dei segni e dei sintomi tipici di quello specifico disturbo. Vi ricordo che segno e sintomo sono due concetti diversi. Il segno è una manifestazione rilevabile e osservabile anche da terze parti (ad esempio, si può notare se una persona è in uno stato confusionale) mentre il sintomo non si manifesta esternamente ma viene percepito e riportato dal soggetto che lo sperimenta (ad esempio è la persona che ci dice di provare un’intensa paura o angoscia verso qualcosa). Il DSM-5 è molto diverso rispetto al precedente DSM IV-TR perché il DSM IV-TR era basato su un sistema categoriale di classificazione mentre il DSM-5 ha abbandonato le categorie per usare un approccio dimensionale che “dovrebbe permettere una descrizione più accurata delle manifestazioni del paziente e aumentare la validità della diagnosi” (DSM-5, p.6). Il DSM-5 si avvale dell’utilizzo di specificatori che rilevano una caratteristica del disturbo e che «forniscono l’opportunità di definire un sottogruppo più omogeneo di individui con il disturbo che condivide determinate caratteristiche» (DSM-5, p.24). Esempi di specificatori sono la gravità del disturbo, la frequenza e la durata di manifestazione dei sintomi (per esempio la durata minima di 6 mesi per molti disturbi presenti nel manuale). Una cosa importante da segnalare è che, per chi usa il manuale ICD, nel DSM-5, prima del nome di ogni disturbo, sono indicati il codice ICD-9-CM, seguito dal codice ICD-10-C. Laddove non ci sia specificato niente vuol dire che per quel disturbo non sono applicabili i codici ICD. Il DSM-5 ci tiene a specificare che i disturbi mentali devono essere inquadrati anche all’interno del contesto sociale, culturale, famigliare e relazionale dell’individuo. Ogni cultura ha le proprie caratteristiche e, per questo motivo, ha dei disturbi che si manifestano con più frequenza rispetto ad altri. Del resto, ogni epoca storica è caratterizzata da disturbi diversi. Se all’epoca di Freud l’isteria nelle donne era un disturbo molto frequente, adesso lo sono l’anoressia e la depressione. Vediamo ora di capire quali sono i vantaggi di usare il DSM-5 e quali, invece, gli svantaggi. Il primo e più importante vantaggio nell’utilizzo del DSM-5 è che, grazie a questo manuale, si utilizza un linguaggio comune tra i diversi professionisti che operano nel settore della salute mentale. Il DSM-5, infatti, consente la condivisione dello stesso strumento da parte dei diversi professionisti della salute come “psichiatri, medici di altre specialità, psicologi, operatori sociali, infermieri, counselor, specialisti nel campo forense e legale, terapisti occupazionali e della riabilitazione, e altri professionisti della sfera sanitaria” (DSM-5, pag. LIII). Inoltre, il DSM-5 è molto utile come “strumento per la raccolta e la comunicazione di statistiche” (DSM-5, pag. LIII) sui disturbi mentali, sulle loro caratteristiche prevalenti, la loro incidenza sulla popolazione e il tasso di mortalità relativo al singolo disturbo. Un altro vantaggio è che non ha un orientamento specifico e questo lo rende ateorico. Questo ultimo vantaggio può, però, allo stesso tempo essere considerato uno svantaggio perché l’ateoricità rende complicata l’attribuzione di un significato ad uno specifico sintomo.
